feat: update prompt.md.template with clearer instructions and agent-specific worklog guidance
- Added instructional note to replace `==` marked sections during task execution - Updated worklog section to specify "Agent: Fill this in as you work on the task" for clearer ownership - Removed redundant instruction about replacing `==` sections that was duplicated in the template
